To be able to prepare a contract for you Musical Moments needs certain information from you. The easiest way to send us that information is for you to fill in the "Event Information Form." By filling it out you are giving us enough information to understand your needs. It can be used for any type of event.

The last page of the form is a "Wedding Music Selection" form. Give us this information if you know it. If you are still in the planning stages, we will get the music selections from you two or three weeks before the date of the wedding. Take a few minutes to listen to us play a selection of pieces by going to our music clips page.

Please fill in this form completely (except for the music selections) so that we will have enough information to quote you an accurate cost for our services. We will contact you right away after we receive this information.

The form is in two different formats. One is in MSWord format, which you can download and fill in with your computer and send to us as an email attachment. The other is in .PDF format which you can download and fill in by hand and mail to us.

Cover Letter

This is a sample cover letter we send with each contract.

Dear Client,

Thank you for choosing Musical Moments to provide the music for your wedding. Please see the enclosed contract. Read your contract carefully, checking the times and locations.

The times indicate playing times, including 30 minutes of prelude music. For example, for a 5:00 pm wedding service, preludes begin at 4:30 pm. The contract will reflect playing times of 4:30 - 5:30 pm. Include accurate directions, please.

Begin now making your music selections and send any special requests to us at least two weeks prior to the wedding. We have a group of preludes that we normally play, but we will include any selections you request. You need to select the pieces that you want played for the entrance of the bridesmaids, groomsmen, family, and bride. You also need to select the recessional(s).

Sample Contract

Musical Moments Music Contracting Service agrees to provide (CLIENT NAME) with a (ENSEMBLE TYPE) on (DATE) from (START TIME) until (END TIME) at (LOCATION). Client agrees to pay Musical Moments a total of (TOTAL AMOUNT) for these services. Client will sign and return this contract with a non-refundable deposit of (DEPOSIT AMOUNT) payable to Susan Maltz/Musical Moments. Receipt of the contract and deposit by Musical Moments will guarantee the date.

THE DEPOSIT IS DUE: (DATE).

THE BALANCE OF: (AMOUNT) IS DUE: (DATE).

In addition, client agrees to provide armless, straight-back chairs for the musicians. Musical Moments must be notified if the performers are expected to play outdoors. If not informed prior to the event, the players reserve the right to refuse service. When performing outdoors the musicians must be in a shaded or covered area. The musicians cannot play in direct sunlight or in damp weather because it is harmful to their instruments.

Adequate lighting is necessary for the musicians to read their music. Candlelight is NOT adequate lighting. The musicians will bring additional lighting when advised.

The musicians will take a 10-minute break every hour.

Important Notice - You must return this contract and your deposit before we will guarantee the date and time. If we do not receive this within 30 days from the date you receive it we reserve the right to release the date and time for another client.
